First Holy Communion Registration Form

Please only fill in this form if you wish your child to be considered for First Holy Communion in 2020, which is open to children who will be at least 7 years old on 1st September 2021.

If you haven't already done so then you will also need to fill in a Parish Registration form.

Your personal details given below will be stored and used by the Parish for the purposes of conducting and administering the Holy Communion preparation course 2021, and for contacting you about youth events in the future. Your details will not otherwise be disclosed outside the parish. Details of how we process your data, and your rights, are on the full Privacy Notice which may be seen in the Parish Office and is on the Diocesan website.
